Medical Services

Appointments

Appointments can be scheduled through our website, by phone, or in person. We request that you provide at least 24 hours' notice if you need to cancel or reschedule an appointment. Failure to provide sufficient notice may result in a cancellation fee.

Payment

Payment is expected at the time of service unless other arrangements have been made in advance. We accept cash, checks, and most major credit cards. We will bill your insurance company for covered services, but you are ultimately responsible for any charges not covered by your insurance.

Insurance

It is your responsibility to verify that we are in-network with your insurance plan. We will make every effort to verify your benefits prior to your appointment, but insurance coverage can change. You are responsible for knowing your coverage and benefits.

Refund and Cancellation Policy

Appointment Cancellations

If you need to cancel or reschedule an appointment, we require at least 24 hours' notice. Cancellations made with less than 24 hours' notice may be subject to a cancellation fee of up to $50. No-show appointments will be charged the full consultation fee.

Payment Refunds

Refunds for medical services may be issued in the following circumstances:

  • Overpayment: If you have overpaid for services, we will refund the excess amount within 30 business days of notification.
  • Insurance Adjustments: If your insurance pays more than anticipated after you have already paid out-of-pocket, we will refund the difference within 30 business days of receiving insurance payment.
  • Billing Errors: If we have made a billing error, we will correct it and issue a refund if applicable within 30 business days of identifying the error.
  • Cancelled Services: If we cancel your scheduled service for any reason, any prepayment will be fully refunded within 14 business days.

Non-Refundable Services

Medical services that have already been provided are generally non-refundable. Once treatment has been rendered, consultation fees, diagnostic testing fees, and procedure fees are not eligible for refund, except in cases of billing errors or duplicate charges.

How to Request a Refund

Please include your name, date of service, invoice number (if available), and reason for the refund request. Refunds will be processed to the original payment method within 30 business days of approval.

Dispute Process

Steps to Dispute a Charge

  1. Contact Our Billing Department: Call (208) 537-1090 or email clinic@eaglerockmedical.com within 60 days of receiving your bill.
  2. Provide Documentation: Include your name, date of service, invoice/statement, and detailed explanation of the disputed charge.
  3. Investigation: We will investigate your dispute within 15 business days and contact you with our findings.
  4. Resolution: If the dispute is resolved in your favor, we will correct the billing error and issue a refund or credit within 30 business days. If we determine the charge is accurate, we will provide a detailed explanation.

Chargeback Policy

If you initiate a chargeback with your credit card company without first attempting to resolve the dispute directly with us, we reserve the right to suspend services until the matter is resolved. We encourage you to contact us first so we can work together to resolve any billing concerns.

Payment Plan Disputes

If you are on a payment plan and have a dispute regarding scheduled payments, please contact our billing department immediately. We will work with you to resolve the issue while maintaining your payment plan terms.

Disclaimer of Warranties
